Prescribing psychiatric medications has historically been guided by symptom clusters, clinical judgment, and a fair amount of waiting. A medication is chosen based on what tends to work for people with similar presentations, you start it, and over weeks or months, you find out whether you're in the group it helps.
The reason this takes so long is that medications affect everyone differently, and a significant part of that difference is genetic. Two people taking the same antidepressant at the same dose can have completely different outcomes, not because one is responding to treatment and one isn't, but because their bodies metabolize the medication differently. One person may clear it too quickly for it to be effective. Another may process it too slowly, allowing it to accumulate and cause side effects. A third may have genetic variations affecting how their brain responds to the medication at the receptor level, regardless of blood levels.

GeneSight is a pharmacogenomic test, which means it looks at genes relevant to how medications work in your body, not at diagnostic markers that tell you whether you have a particular condition. The distinction matters: GeneSight doesn't diagnose depression, anxiety, ADHD, or anything else. What it does is give your provider information about gene-drug interactions that can affect medication selection and dosing.
The test analyzes genes in two main categories.
Pharmacokinetic genes are the ones that affect how your body processes medications. The key players are enzymes in the cytochrome P450 family, particularly CYP2D6 and CYP2C19. These enzymes are responsible for metabolizing a large portion of psychiatric medications. Genetic variations in these enzymes make some people poor metabolizers (the medication builds up), ultra-rapid metabolizers (the medication clears before it can work), or anything in between. The GeneSight report shows where you fall on that spectrum for the specific medications you're being considered for.
Pharmacodynamic genes affect how your brain responds to medications at the receptor and transporter level. One of the most relevant is the SLC6A4 gene, which encodes the serotonin transporter. Variations in this gene can affect how you respond to medications that target the serotonin system, including SSRIs.
A narrative review published in PMC notes that clinical guidelines from major pharmacogenomics organizations have now established recommendations for 14 antidepressants, 7 antipsychotics, and 3 mood stabilizers based on pharmacogenomic data. GeneSight translates this evidence into a clinical report organized around the medications your provider is considering.
The GeneSight report organizes medications into three categories based on gene-drug interactions.
Use as directed means the available genetic data does not indicate a significant interaction likely to affect how you respond to this medication.
Use with caution means there is a potential gene-drug interaction that your provider should be aware of, and monitoring may be warranted.
Use with increased caution and more frequent monitoring means there is a significant interaction that may affect efficacy or increase the likelihood of side effects.
This doesn't tell your provider which medication to prescribe. It gives them additional information to weigh alongside your symptoms, your history, what you've tried before, and your preferences. The goal is a more informed starting point, not a genetic prescription.
According to a systematic review of the GeneSight test published in PMC, patients who received GeneSight-guided care showed greater improvements in depression symptoms and higher rates of response compared with patients receiving treatment as usual. Research also shows that pharmacogenomic-guided prescribing can reduce the number of medication trials a patient goes through.
What it cannot do is guarantee a medication will work. Psychiatric medications are affected by many factors beyond genetics, including the diagnosis itself, co-occurring conditions, lifestyle factors, other medications, and the therapeutic relationship. Genetics is one piece of a complex picture, and GeneSight is designed to inform prescribing decisions rather than replace clinical judgment.
It also doesn't test for every gene or every medication. The report covers the medications most commonly prescribed for psychiatric conditions, and the science is still evolving for some gene-drug interactions.
GeneSight is particularly useful in several situations.
If you've tried multiple psychiatric medications without finding one that works or that you tolerated, testing can help explain why and point toward alternatives less likely to have the same problem.

A review of pharmacogenomics in psychiatry practice published in PMC supports the clinical value of pharmacogenomic testing, particularly for patients with treatment-resistant depression and those on complex medication regimens.
